NYITCOM Alumni Awards Nomination Form

The Dean’s Committee of the College of Osteopathic Medicine is accepting nominations for its annual Alumni Awards event. Alumni and friends are welcome to nominate themselves or classmates and colleagues, and are welcome to nominate more than one individual. All submissions will be added to a running list and will be reviewed by the Dean’s Committee. Categories could include, but are not limited to:

DISTINGUISHED ALUMNI:
The Distinguished Alumnus(a) Award honors alumni who have personified the NYITCOM tradition of excellence and brought credit to the COM through their personal accomplishments, professional accomplishments, or humanitarian service. The contributions do not need to be publicly renowned but should represent important creative effort or accomplishment with societal or educational value. The award is granted to an alum in recognition of his or her devotion and loyal support in the advancement of his or her college.

ALUMNI OF THE YEAR:
The Alumnus(a) of the Year award is given to an alum who has distinguished himself or herself professionally or shown considerable support to NYITCOM. This award is presented to alumni who have achieved distinction in their field, or have demonstrated loyalty to, and an active interest in NYITCOM.

YOUNG ALUMNI:
This award honors and recognizes an alumnus(a) who has graduated within the past 10 years. This award is presented to an alumnus(a) who has distinguished themselves beyond others by providing either outstanding service to NYITCOM or has made an outstanding contribution to their profession or community.

EXCELLENCE IN MEDICINE:
The Excellence in Medicine Award recognizes an alumnus(a) physician who exemplifies the highest values of volunteerism, community engagement, leadership, and dedication to the care of underserved populations.

COMMUNITY PARTNER:
The Community Partner is one who enhances the image of NYITCOM or the medical profession through various activities. These activities include, but are not limited to, community service, legislative activities, participation in organizations affecting healthcare and/or contributions to education and research.

HONORARY ALUMNI: The Honorary Alumnus(a) is given to friends of New York Tech in recognition of outstanding service to NYITCOM.
